Job Description

  • Assess patients based on:
  • Interview of patient and significant other
  • Chart review
  • Referrals from other health team members and outside agencies.
  • Provide counseling and teaching for clinic patients and their family/significant others who are new or referred by the physician.
  • Provide the appropriate nursing interventions based upon assessment which will include:
  • Instructing patients and/or family members/significant others regarding treatment, medication, diagnostic procedures and the importance of follow-up visits.
  • Interpreting the patient's understanding and emotional response to their condition.
  • Observing and identify underlying and contributing factors/concerns, especially socioeconomic, cultural, physical and language barriers.
  • Assisting patients and/or significant others to establish a medication schedule when on multiple medications.
  • Making referrals to other departments and agencies as appropriate, i.e. Home Care, Visiting Nurse Service, Department of Health, Social Service, Dietary, etc.
  • Provide health teaching and counseling to the patient and/or significant other.
  • Triage patients who present to clinic without appointments.
  • Make appropriate referrals
  • Communicate with physician or other health care members as indicated.
  • Document all observations on one or more of the following:
  • Initial Nursing Assessment
  • Follow-up check off list/flow sheet
  • Assist with hospital admission process.
  • Expedite process by communicating with necessary personnel.
  • Support patient and family members.
  • Participate in preventative health measures and case findings.
  • Maintain health maintenance status of patient by review of necessary records.
  • Diagnose human response to actual or potential health problems at the earliest signs and symptoms and initiate treatment and/or make referrals for appropriate care.
  • Identify the family members and/or significant other of patients with an infectious process and encourage testing and prophylaxis as appropriate, i.e. tuberculin skin testing, hepatitis screen.
  • Review immunization records to update status.
  • Identify those patients who are at risk for certain diseases which could be minimized with appropriate vaccines, i.e. influenza, pneumovax.
  • Conduct heath education classes to promote wellness, i.e. smoking cessation classes.
  • Communicate patient care problems/issues via the proper channels.
  • Inform Head Nurse of patients with complicated problems and those who are awaiting admission.

  • Accepts full responsibility and accountability of unit operations.

2N Receives rcpot1 by making walking rounds with incoming/outgoing staff.

3􀀊 Checks for absent and late employees and monitors the sign in and sign out sheets for accuracy in documentation of attendance.

4r Ensures staff compliance with daily morning/evening briefs/huddles during shift change.

  • - Ensures staff compliance with patient-centered intershift reporting that includes the patient in handoff communications, as well as updating the bedside patient whiteboard information.

6i Ensures staff utilization of SBAR for all handoff patient communications.

  • Delegates' staff to perform functional assignments based on individual competency and patient skill mix.
  • Delegates assignments based on staff competence and priority of patient care utilizing the patient classification system and follow up.
  • Conducts patient rounds on the unit daily with Service Excellence initiatives as a top priority.

i0r Fo11ow-up with all work requests to ensure needs of the unit arc met timely.

  • Supervises medication treatment and diagnostic procedures administered or perfo1111ed by the staff.

1ec Performs daily environmental unit rounds utilizing Joint Commission compliance resource tools, and to assure compliance with infection Control and Safety Policies.

rir Ensures unit operations are in compliance with all Joint Commission and Regulatory Agency requirements.

cti Facilitates a culture of continuous quality and perfomrnnce improvement using the Breakthrough A3 model of performance improvement for the organization.

n,N Works with Nursing Quality to ensure staff compliance with unit, organizational and regulatory agency policy and procedures.

  • Role models and supports organizational strategic priorities in an enthusiastic and positive manner.
  • Ensures staff compliance with patient-centered hourly rounding utilizing the 6 Ps.
  • Submits all reports and assignments by instructed dates.
  • Coordinates patient care by collaborating with the multidisciplinary team.
  • Communicates effectively with the patientifamily, medical, nursing and other staff.

21e Escalates changes in patient's medical condition to the Interdisciplinary team.

22a Maintains standards of care by ensuring that an individual plan of care for each patient is developed and implemented.

  • Supervises and assists the staff in patient assessments, documentation, implementation and evaluation of Nursing Care Plan in the Electronic Medical Record.

24t Ensure staff compliance with the implementation of Patient/Family teaching upon admission, and ensures continuity of patient/family education throughout hospitalization.

25f Daily supervision of all staff.

  • Ensures unit supplies are adequate to meet all patient care needs.
  • Completes performance evaluation for assigned staff, and promotes staff development and continuing education.

28t Serves as clinical resource person for staff􀆌 and assumes patient care responsibilities as the need arises.

29t Completes time schedule for staffing and submits timely to the ADON/AON for approval.

  • Participates in the development and in the implementation of unit orientation to all new employees.

3ea Counsels staff in collaboration with the ADON/AON, and makes recommendations for further corrective action.

32a Keeps anecdotal records of significant occurrences on the unit of staff behavior and performance issues.

33a Attends and pmticipates in committee meetings as assigned.

3ma Conducts monthly staff meetings to communicate all unit, organizational and regulatory agency updates and or changes to policies/procedures.

35a Actively participates in pilot studies to improve unit and organizational operation.

  • Maintains clinical competencies by attending continuing education pro6rrams.

37a Responsible for repot1ing to the ADON/AON any situation or problem on the service.

38e Completes end of shiH report and provides a handoff to the AON coming on duty.

39a Participates and suppo11s Team Building, Problem Solving, and Breakthrough Initiatives to enhance unit and organizational operations.

m0m Uses the Breakthrough A3 model for all performance improvement projects on the unit.

mee Identifies and engages staff to promote safety, quality, and performance improvement on the unit.

m2m Performs all related duties as assigned.

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Special Note

For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
