Join to apply for the Oncology Nurse Navigator role at Beacon Health System Responsibilities Manage resources through interdisciplinary collaboration to achieve optimal patient outcomes for oncology patients. Coordinate research data, community physicians, and Beacon Health System resources into a seamless access and care model. Serve as a hospital and Beacon Medical Group ambassador from the patient's first contact to follow-up post-treatment. Provide education on services available to oncology patients across specialty departments. Develop survivorship care plans and provide teaching and resources for survivorship care. Assist adult oncology patients in navigating the healthcare system. Conduct initial discussions with newly diagnosed patients and coordinate care plans. Network with physicians and healthcare providers to implement patient care plans. Support patients with issues such as insurance, transportation, and language barriers. Initiate referrals to community resources and coordinate with community agencies. Utilize interdisciplinary care conferences for patient intervention planning. Encourage patient compliance and report variance trends. Develop and discuss survivorship plans with patients and families. Participate in community health screenings and educational offerings. Maintain active role in the Oncology Care Committee. Participate in Continuous Quality Improvement activities. Manage activities of multidisciplinary oncology clinics. Complete other assignments and special projects as directed. Identify eligible patients for survivorship activities and develop care plans. Ensure organizational compliance with policies, certifications, and safety standards. Participate in organizational initiatives such as The Beacon Way. Maintain necessary licenses, certifications, and competencies. Work in various environments, including travel to different locations. Perform physical functions required for the role. Qualifications Graduate of an accredited nursing program with current RN license in Indiana. Oncology Certified Nursing (OCN) certification required or to be obtained within two years. Minimum of five years of oncology nursing experience preferred. Leadership experience in healthcare preferred. Skills and Knowledge Thorough knowledge of oncology care practices and community resources. Effective counseling, communication, and interpersonal skills. Strong time management and problem-solving skills. Proficiency with computer programs used in healthcare settings. Additional Details Work hours may vary based on patient needs.
